On the second page of the questionnaire, answer YES if you are 65 or older, or have comorbidities:
Older adults (65+) or adults with one or multiple comorbid conditions including not limited to asthma, heart disease, hypertension, obesity, cardiovascular disease, diabetes mellitus, chronic lung, liver or renal disease, cancers, suppressed immune systems, who are at high risk of mortality and severe morbidity resulting from COVID infection.
Answer YES to the question: Are you a critical infrastructure worker in an “essential business/industry” as specified in the Governor’s 3rd executive order, and found at https://oklahoma.gov/content/dam/ok/en/covid19/documents/vaccine/Oklahoma%20Essential%20Industries%20List%20-%201-8-21.pdf
You will receive an email with what phase you are in, as well as an email when to schedule your registration when your phase/group becomes active.
